Authorization for Prescription Medication Form E-33
Prescription Medication:
In keeping with state regulations, school board policy requires that an "Authorization to Administer Prescription Medicine" Form be completed by the subscribing doctor and brought in by the parent along with the medication. This form (PSDSB E-33) is available from our attendance office, phone 312-4938 or can be printed from this page. When dropping off the medication, the parent may need to complete another form as well. A parent note sent with the medication will be kept under lock and key and must be in the original pharmacy bottle with the student's name on the label. Parents should keep non-school hour doses of medication at home, so the bottle can remain locked at school. *Please see form PCSB E-41 & 42 for insulin administration procedures for diabetics.

This form (PCDSB Form E-34) is available from our attendance office, phone 312-4938 or can be printed from this page. A parent note sent with the student is not sufficient, and only the parent may bring in the medication. The medication will be labeled and kept under lock and key. Parents should keep non-school hour doses of mediation at home, so the medicine can remain locked at school.
